What to have ready

Your insurance card (call or text ahead to begin verification — if we need an image, our team will provide the approved way to send it securely), a list of medications — the interactions matter to your dentistry more than most people expect — and the name of your previous dental office if you'd like records transferred. One signed release and we handle the chasing.

For children, whichever parent books is enough; bring any orthodontic or specialist history and we'll fold it into the chart.

The health-history part, honestly

The form asks about your whole health because your mouth participates in it: medications that dry it out, conditions that change healing, anxieties that change pacing. Answer plainly — including the it's-been-a-while confession and the I-hate-dentists one. Both change how we plan your visit, in your favor.

Can I fill out forms before I arrive?
Yes — call the front desk and they'll get paperwork to you ahead of the visit, or arrive ten minutes early and it's handled at the desk with help on hand.
What if I don't know my insurance details?
Text a photo of the card and stop worrying — decoding coverage is the front desk's daily sport, and you'll know what's covered before your appointment.
Do I need my old records to book?
No — book first, and sign a release when you're in. Recent X-rays are worth transferring; everything else we can rebuild from the exam.
